Your clinician would like to know how you are doing and has therefore asked you to complete questionnaires on the KLIK website. You complete the questionnaires before the appointment in the hospital. The clinician can view your answers on the questionnaires and discuss this with you during the consultation. By this, the clinician knows how you are doing and can offer you the right help.
